GRADE 7 Application Procedure and Requirements

GRADE 7 APPLICATION PROCEDURE AND REQUIREMENTS FOR SY 2018-2019
 
1.  Download the Application for Admission form from the link at the bottom of this page
     and print all pages of the forms on size A4 bond paper.  The applicant should accomplish
     the form completely in his own handwriting.
 
2.  Submit the form together with all of the following requirements:

          a.  Photocopy of the applicant’s final, completed Grade 5 report card (SY 2016-2017) –bring original document
               for verification

          b.  Photocopy of the applicant’s Grade 6 report card for the first grading period (SY 2017-2018) – bring original document
               for verification

          c.   Handwritten autobiography on size A4 bond paper
          d.   Two recent 1”x1” ID pictures in color (one ID picture should already be glued or pasted to the application form and the other
                should be given when submitting the form)
          e.   Photocopy of the birth certificate (NSO copy)
                   If the applicant is not a Filipino citizen, submit a photocopy of his
                     · Alien Certificate of Registration (ACR), and
                     · Special Study Permit issued by the Bureau of Immigration
 
                   If the applicant has dual citizenship, submit a photocopy of
                     · his Philippine passport OR
                     · a Certificate of Recognition as a Filipino Citizen.
 
          f.   The last page of the Privacy Policy and Terms of Agreement form. Consent to the Privacy Policy and Terms of Agreement
               is a condition for admission.
 
3.  Pay the non-refundable amount of Php 700.00 application and testing fee at the High School
     cashier.  The cashier’s office hours are from 7:00AM to 3:00PM, Mondays to Fridays. Please note
     that all offices in the AJHS will be closed on July 31, 2017 in observance of the feast of St. Ignatius.
     For expatriates and repatriating Filipino applicants, an additional processing fee of US$25.00 will be
     required.
 
4.  Present the (a) Official Receipt, (b) fully accomplished Application Form, and (c) complete set of
     requirements to the High School Office of Admission and Aid to claim the Exam Permit for the Ateneo
     High School Entrance Test on or before 12:00 noon on Monday, October 9, 2017.
 
     A representative may submit the application documents on behalf of the applicant, but the submission
     should be done in person to the High School Office of Admission and Aid.
 
IMPORTANT:  Incomplete application forms will NOT be processed.  If the report card for the first grading period for Grade 6 is not yet available by the application deadline, the photocopy may be submitted as soon as the report card is issued.  All other documents, however, must be submitted on or before the deadline. 

The schedule of the AHSET for Grade 7 is as follows:
 
            Session 1 – Saturday AM, October 21, 2017
            Session 2 – Saturday PM, October 21, 2017
            Session 3 – Sunday AM, October 22, 2017
 
The results of the AHSET will be released on or before 31 January 2018.
