Application Requirements and Procedures

 
  1. Before proceeding, please click on this link and read the ADMU Privacy Policy for Applicants, Students and Alumni.   
  1. Download the Application for Admission form from the link at the bottom of this page and print all pages of the forms on size A4 bond paper. (Note that A4 is different from short or legal sized bond paper.) 
  1. Accomplish the form completely in your own handwriting and submit the same together with the following requirements:
a.      Photocopy of your final, completed Grade 9 report card (SY2017-2018)
b.      Photocopy of your Grade 10 report card for the 1st Grading Period (SY2018-2019)
c.      Autobiography - handwritten or typewritten on size A4 bond paper (1-2 pages only)
d.      Two recent I.D. pictures (1” x 1”) in COLOR (One ID picture should already be  attached to the application form and the other should be given when submitting the form.)
e.      Photocopy of Birth Certificate (Philippine Statistics Authority Copy)

         - If applicant is NOT a Filipino citizen, submit a photocopy of your
           i.     Alien Certificate of Registration (ACR), and
           ii.    Special Study Permit issued by the Bureau of Immigration

         - If applicant has DUAL citizenship, submit a photocopy of your Philippine Passport  or a Certificate of Recognition as a Filipino Citizen.

f.      Two Letters of Recommendation from the last school attended (The persons recommending you may choose to submit recommendation letters OR may fill out the Recommendation Form at the end of the Application Form.)
g.      PLACE THE APPLICATION FORM ALONG WITH THE REQUIREMENTS IN A LONG BROWN ENVELOPE, THEN PASTE THE COVER PAGE OF THE APPLICATION FORM CONTAINING THE CHECKLIST OF REQUIREMENTS ON THE OUTER SIDE OF THE ENVELOPE (the flat side without the flap).
NOTE: Applicants enrolled abroad will be required to submit a certificate of eligibility to Grade 11 issued by the Philippine Department of Education upon acceptance and enrollment.
  1. Present the requirements to the High School Admission and Scholarships Office for checking of completeness and issuance of charge slip. 
  1. Pay the non-refundable application and testing fee of Php750.00 at the High School Cashier’s office.
The Cashier only accepts cash payments for the application and testing fee.
Cashier’s office hours - 7:00AM to 3:00PM, Mondays to Fridays.

  • Applicants from public schools need to present a letter from their principal certifying that (a) they belong to the Top 10, and (b) they are financially challenged in order for their application and testing fees to be waived.
  •  For expatriates and repatriating Filipino applicants, an additional processing fee of US$25.00 will be charged.
  1. Present the (a) Official Receipt, (b) fully accomplished Application Form, and (c) complete set of requirements to the High School Admission and Scholarships Office Staff to claim the Entrance Test Permit  for the Ateneo Senior High Admission and Placement Exam (A-SHAPE)  before the DEADLINE
12:00 noon on Friday, September 7, 2018 for applicants from the AJHS
12:00 noon on Friday, September 14, 2018 for all other applicants

 
  7. Submit the application documents as soon as possible in order to be able to choose your
      testing session. This will be indicated in the exam permit. The schedule of the
      A-SHAPE is as follows:

 
  1. Session 1- Saturday, October 6, 7:30 AM - 12:00 NN        
  2. Session 2- Saturday, October 6, 1:00 PM - 5:30 PM
  1. Session 3- Sunday, October 7,  7:30 AM -12:00 NN           
  2. Session 4- Sunday, October 7, 1:00 PM - 5:30 PM
 Slots per testing date are limited and will be given on a first come-first served basis.  Applicants who submit their requirements nearing the deadline may not get their first choice of testing slot. 

  
Important Reminders

  1. A representative may submit the application documents on behalf of the applicant, but this  representative has to personally go through all of the steps in the process (inspection of requirements, payment, and claiming of the exam permit).  Requirements left at the front desk or sent by courier service will NOT be processed.
  2. Please make sure that the application form is completely filled up and that all requirements are complete before coming to submit these requirements.  If the report card for the 1st term for Grade 10 is not yet available by the application deadline, the photocopy may be submitted as soon as the report card is issued.  All other documents, however, must be complete for the application to be processed.
