Application Requirements and Procedures

1.      Download the Application for Admission form from the link at the bottom of this page and print all pages of the forms on size A4 bond paper. Accomplish the form completely in your own handwriting and submit the same together with the following requirements:
a.      Photocopy of your final, completed Grade 9 report card (SY2016-2017)
b.      Photocopy of your Grade 10 report card for the 1st Grading Period (SY2017-2018)
c.      HANDWRITTEN autobiography on size A4 bond paper
d.      Two recent I.D. pictures (1” x 1”) in COLOR (One ID picture should already be attached to the
          application form and the other should be given when submitting the form.)
e.      Photocopy of Birth Certificate (NSO Copy)
·        If applicant is NOT a Filipino citizen, submit a photocopy of your
       i.  Alien Certificate of Registration (ACR), and
       ii.  Special Study Permit issued by the Bureau of Immigration.
·        If applicant has DUAL citizenship, submit a photocopy of your Philippine Passport or
         a Certificate of Recognition as a Filipino Citizen

f.       Two Letters of Recommendation from the last school attended (The persons recommending
          you may choose to submit recommendation letters OR may fill out the Recommendation Form at
          the end of the Application Form.)

g.      The last page of the Privacy Policy and Terms of Agreement form.  (Consent to the Privacy
          Policy and Terms of Agreement is a condition for admission.

h.      PLACE THE APPLICATION FORM ALONG WITH THE REQUIREMENTS IN A LONG BROWN
         ENVELOPE
,THEN PASTE THE COVER PAGE OF THE APPLICATION FORM CONTAINING
         THE CHECKLIST OF REQUIREMENTS IN FRONT OF THE ENVELOPE.
 
2.      Pay the non-refundable amount of Php750.00 application and testing fee at the High School cashier. Cashier’s office hours are from 7:00AM to 3:00PM, Mondays to Fridays. Please note that all ASHS offices will be closed on July 31, 2017 in observance of the feast of St. Ignatius. Applicants from public schools need to present a letter from their principal certifying that (a) they belong to the Top 10, and (b) they are financially needy in order for their application and testing fees to be waived. For expatriates and repatriating Filipino applicants, an additional processing fee of US$25.00 will be charged.
 
3.      Present the (a) Official Receipt, (b) fully accomplished Application Form, and (c) a complete set of requirements to the High School Admission and Aid Office Staff to claim the Entrance Test Permit  for the Ateneo Senior High Admission and Placement Exam (A-SHAPE) on or before 2:00 p.m. on Monday, September 18, 2017. Submit the application documents as soon as possible so you may choose your testing session. Again, please note that all ASHS offices will be closed on July 31, 2017 in observance of the feast of St. Ignatius. The schedule of the A-SHAPE for Grade 11 is as follows:
a.      Session 1- Saturday, October 14 AM          b.  Session 2- Saturday, October 14 PM
c.      Session 3- Sunday, October 15 AM             d.  Session 4- Sunday, October 15 PM
 
       A representative may submit the application documents on behalf of the applicant, but the  
       submission should be done in person to the High School Office of Admission and Aid.
(d) IMPORTANT: Incomplete application forms will not be processed.  If the report card for the 1st term for Grade 10 is not yet available by the application deadline, the photocopy may be submitted as soon as the report card is issued.  All other documents, however, must be submitted on or before the deadline.
