IDENTIFICATION CARD SERVICES

     The Office of the Associate Dean for Student Affairs is in charge of the production and release of identification cards for legitimately enrolled students for the current school year, as well as all students’ drivers, chaperons and bodyguards who regularly come to the campus.


Guidelines of the I.D. Policy:

1. All students must get a new I.D. card at the beginning of every school year. A student I.D. card is valid only for one school year. The school year is prominently printed on the I.D. card itself. Every school year begins on the 1st of April and ends on the 31st of March the succeeding year.

2. The color-coding for the different year levels are as follows:

          a. Freshmen: GREEN
          b. Sophomores: RED
          c. Juniors: LIGHT BLUE
          d. Seniors: NAVY BLUE
          e. Graduate School/Non-degree: YELLOW

3. An I.D. card is considered valid only if it has the correct color-coded Validation Sticker per semester.

4. Care must be taken to keep the Validation Sticker intact for the entire semester for which it is valid.

5. The cost of an I.D. card is P 100.00 for the first I.D. card issued to a student for the school year. The cost of the second I.D. card issued to the student for the school year is P 200.00. The cost of the third and subsequent I.D. cards is P 300.00.

6. Students are not prevented from securing an extra I.D. card if they so desire. However, care must be taken to ensure that it is not used by other persons. Misrepresentation merits severe consequences.

7. Loss of an I.D. card, including the extra I.D. card, must be immediately reported to the office as a security measure.


Steps for New I.D. Card Application:

1. Present proof of current enrollment as a student (i.e., tuition fee receipt and/or registration form) to the Office of the Associate Dean for Student Affairs.

2. Secure and fill up two copies of the application form

          a. Print name legibly.
          b. Put the I.D. number.
          c. Have picture taken in the office or other designated venue.
          d. Claim I.D. by presenting the temporary I.D.


Steps for Replacement of Lost/Stolen I.D. Card:

1. Secure Charge Slip from the Office of the Associate Dean for Student Affairs.

2. Proceed to the Cashier to pay for the cost of the I.D. card (P 200.00 for the second I.D. card for the school year, P 300.00 for the third and subsequent I.D. cards)

3. Present a notarized affidavit of loss.

4. Secure and fill up two copies of the application form

          a. Print name legibly.
          b. Put the I.D. number.

5. Have picture taken at the office or other designated venue.

6. Claim I.D. by presenting the temporary I.D.


TEMPORARY ID

     Temporary ID's maybe issued to students who left their ID's at home. The students concerned must pay a penalty fee of P30.00. Slip will be issued and paid at the cashier's office.

     A certificate of enrollment issued and valid for only one day.

     A regular temporary ID form is issued during registration period for those applying for new ID's. Duration or validity is specified in the form itself.

Steps:

1. Secure charge slip from the Office of the Associate Dean for Student Affairs.

2. Proceed to the cashier to pay the cost of P30.00.

     Claim temporary ID at the Office of the Associate Dean for Student Affairs stamped with date of validity.


APPLICATION FOR BODYGUARD'S/DRIVER'S ID

A student may register his/her official chaperons on campus (e.g., bodyguards, drivers, etc.) subject to the regulation pertinent to the request as stipulated in the Student Handbook (please refer to page 23 of section #6 of the 35th edition of the Handbook).


Steps:

1. Request bodyguard/chaperon/driver to report to Associate Dean for Student Affairs office. He/she must have the following:

          a. Proof of employment
          b. License or any other ID

2. Secure a charge slip and pay an ID fee of P80.00 at the Cashier's office.

3. Fill-up an application form at the Associate Dean for Student Affairs office and have ID picture taken at the office.

4. Claim ID at designated date of pick-up.
