CERTIFICATE OF GOOD MORAL CHARACTER


Steps in Securing a Certificate of Good Moral Character:

1. Pay a processing fee of P10.00 per request. (Maximum of 8 copies per request)

2. Accomplish the Request Form for a Certificate of Good Moral Character from the Office of the Associate Dean for Student Affairs

     a. Print name, year & course and I.D. number legibly.
     b. Indicate the purpose of the request.

3. Claim the Certificate of Good Moral Character one day after the request.

For requests for more personalized Certificates of Good Moral Character, the following must be submitted:

     • Letter of Recommendation
     • Photocopy of Transcript of Records
     • Resume
     • 1x1 I.D. picture
