THEFT REPORT

When a theft occurs, students must immediately report the incident to the Office of the Associate Dean for Student Affairs. Complete details must be provided (who, what, where, when, how, why, etc.) for the necessary action to be taken immediately.


Steps in Filing a Theft Report:

1. Students who wish to file a Theft Report may get a form at the office.

2. Fill-up the Theft Report with complete details and accounts of the incident immediately after its occurrence.

3. Submit to the Office of the Associate Dean for Student Affairs.

4. The details are then posted at the Theft Board of the Office of the Associate Dean for Student Affairs along the EDSA walk.

5. The same information is likewise forwarded to the Security Office, the Department of Student Welfare and Services and the University Physical Plant for proper action.
