Re: Health Requirements prior to enrolment
Dear LS Students,
Welcome back to the LS Campus in the coming semester! Please be reminded to prepare for enrollment with the following health requirements to avoid problems in enlisting:
1. Updating of Blue PHR should be done from the period after the release of grades and before enrolment (simultaneous with the required updating of AISIS information): visit your BluePHR personal record to include any recent vaccinations through the link bluepass.ateneo.edu/auth/login. If there are no changes that you need to update on your health record, you will still need to update your record prior to enrolment by scrolling to the bottom of the record and click on the verification that you have updated your record. You may check the memorandum from the Vice President of the Loyola Schools, Dr Maria Luz Vilches, memo instructing students to regularly update their BluePHR n compliance with the CHED guidelines on reporting updated vaccination status: (https://sites.google.com/ateneo.edu/ls-one/ls-memos?authuser=0#h.g3tbywesnvpj). If you have not yet created your personal health record, please do so prior to the enrollment date.
2. First-year, second-year, and third-year students who have not yet complied with the requirement for a Chest Xray and completed immunization records in their online medical record should upload this. Submission of these requirements will be needed for enrollment. If you have already submitted this, there is no need to re-submit.
3. If you have declared comorbidities in your Blue PHR and wish to enroll in any onsite classes, including Hybrid or Flex, you will have to submit a medical certificate of stable comorbidity and accomplish the following form signifying your agreement with on-site classes at the following link: https://go.ateneo.edu/Undertaking-Students-SignificantComorbidities
4. Philhealth insurance is no longer a requisite for enrollment as of the CHED Memorandum Order 05 s. 2022, published 30 May 2022.
Note that the requirements for updating your health records have been a recurring reminder. We look forward to your cooperation to avoid any difficulties in the coming enrollment period. Thank you for doing your part in keeping the campus safe.
